Due to upcoming maternity leave the Hospice has a temporary opportunity in the PACE (Palliative Advice Centre) Team.

We are looking to recruit an experienced clinical colleague with a passion for delivering exemplary care, to join us in looking after people affected by life-limiting illness in both the Hospice and community settings.

You will be working mainly in our Palliative Advice Centre, as well as being an integral part of the medical team on the in-patient unit. Our services are based in our purpose-built hospice situated one mile from Macclesfield town centre. There is ample free parking available on site.

Our new team member will:

- Deliver expert advice to healthcare professionals, patients and relatives both in person and over the phone

- Undertake face to face reviews of palliative patients, working across clinical settings, inclusive of Hospice & community work

- Coordinate care for people with a life-limiting illness

- Be confident liaising with other healthcare professionals, making recommendations for changes to medication regimes/plans of care and advocating for patients and their families

- Be proactive in-service development Deliver evidence based, quality care
