Due to an exciting expansion of services, the Hospice is adding to their well-established medical team.

We are seeking to recruit an experienced Advanced Clinical Practitioner with a passion for delivering exemplary care, to join us in looking after people affected by life-limiting illness in both the Hospice and community settings.

You will be working in our newly developed Palliative Advice Centre (PACE), as well as being an integral part of the medical team on the in-patient unit and Sunflower Living Well Centre.

Our services are based in our purpose-built hospice situated one mile from Macclesfield town centre. There is ample free parking available on site.

PACE operates 24/7. You will be expected to be able to work across 7 days, between the hours of 8am and 8pm.

Our new team member will:

• Undertake face to face reviews of palliative patients, working across clinical settings, inclusive of hospice & community work
• Deliver expert advice to healthcare professionals, patients and relatives both in person and over the phone
• Coordinate care for people with a life-limiting illness
• Be confident liaising with other healthcare professionals, making recommendations for changes to medication regimes/plans of care and advocating for patients and their families
• Be proactive in-service development
• Deliver evidence based, quality care
